Please use the online appointment booking system ("Patient portal")

We release appointment dates one to two weeks in advance and try and keep some same day availability open. 

If you are having trouble accessing or booking appointments please email us and we will walk you through the steps.

If you need to call please leave a message and we will get back to you when we are able.

There will be different appointment lengths available.

If there is only one issue that you would like dealt with then please book for a regular appointment.

 

If there are two to three issues then please book an extended appointment.

Daily Cancellation List

Only available to current patients of the clinic

We offer a "cancellation list" which allows us to contact people on a non-urgent basis for quick questions (<5 mins) when we have openings during the day. This is best used by patients who have a flexible schedule and are available for a phone call. 

If you would like to be placed on today's cancellation list please email us cancellation@ballemmedical.com with cancellation list in the subject line and a brief reason (refill, result review, etc) in the message. Please do not include medical information.

No guarantees are made that you will be called and if you need more than 5 mins please book an appointment.

If we call and are unable to get ahold of you this does NOT count as a no-show, however, if you sign up and are unable to be reached more than 3 times we will not allow you to use the cancellation list.

After Hours Care

For emergency care please visit the nearest hospital or call 9-1-1. 

 

If you have general health concerns/questions, HealthLink is a recommended number to call for 24/7 health advice or resources.  1-866-408-5465 or 8-1-1

If Dr Ballem is not available and you feel that you need same day care, please go to an urgent care clinic convenient to you. Please request that they send us a copy of the note so we can follow up as needed. 

If this is a non emergency and cannot reasonably wait until business hours please call the clinic number and leave a message.
